Вопросы лечения стриктур уретры относятся к наиболее сложным и акту-альным разделам оперативной урологии. Именно поэтому к ней неоднократно были обращены научные и практические работы ведущих российских специалистов в реконструктивной урологии таких как Хольцов Б.Н. [42], Русаков В.И. [38], Лоран О.Б. [16;31;32], Красулин В.В. [26], Коган М.И. [19-25], Лопаткин Н.А. [30], Даренков С.П. [4], Трапездникова М.Ф. [40;41] и Щеплев П.А. [43], так и экспертов в эндоскопической урологии – Мартова А.Г. [33-35], Камалова А.А. [18] и многих других.
На сегодняшний день в виду урбанизации и индустриализации общества отмечается рост заболеваемости стриктурами уретры в связи с транспортным травматизмом. Согласно мнению некоторых авторов, в будущем закономерно увеличение количества больных стриктурами уретры в связи с повсеместным распространением эндоуретральных методов лечения по поводу ДГПЖ и мочекаменной болезни. Все более частой причиной стриктур уретры является специфическое кожное заболевание — лихен склероз. Инфекции, передаваемые половым путем и, прежде всего, гонорея, все реже являются причиной стриктуры уретры, т.к. они успешно лечатся антибиотиками [56].
Отечественными и зарубежными урологами предложены различные кон-сервативные и хирургические методы лечения стриктур мочеиспускательного канала такие, как бужирование, внутренняя оптическая уретротомия, имплантация постоянных уретральных стентов и реконструктивно-восстановительные операции [231;262].
Изучение роли спонгиофиброза - ведущего процесса в формировании стриктуры - позволило объяснить причины неэффективности бужирования уретры, и оценить результаты внутренней оптической уретротомии, в ходе которой не выполняется радикальное удаление склеротически измененных тканей мочеиспускательного канала. Становиться очевидным, что единственным патогенетическим обоснованным методом лечения стриктур уретры является уретропластика, при которой сужение иссекается в пределах здоровых тканей [87].
Однако не существует универсальной хирургической техники, приемлемой для различных видов стриктур, что обязывает урологов, занимающихся данной проблемой, владеть различными методами уретропластики и применять из их большого разнообразия наиболее подходящий вариант в соответствии с конкретной клинической ситуацией [6].
Основным достоинством открытой хирургической коррекции стриктур уретры представляется значительно меньшая частота рецидивов заболевания по сравнению с альтернативными малоинвазивными методиками. Устранение рубцового сужения и восстановление проходимости просвета уретры «любой ценой» зачастую влечет за собой те или иные нежелательные последствия, которые принимают еще более тяжелый характер у пациентов, перенесших неоднократные оперативные вмешательства.
Цель работы – повышение эффективности лечения мужчин, страдающих стриктурами мочеиспускательного канала или дистракционным дефектом уретры после перелома костей таза.
Для достижения поставленной цели предстояло решить следующие задачи:
1. Определить основные этиологические факторы возникновения стриктур уретры у мужчин.
2. Оценить эффективность лечения стриктур уретры с использованием раз-личных методик уретропластики.
3. Определить показания и противопоказания к методикам уретропластики в зависимости от пересечения спонгиозного тела или без такового.
4. Провести анализ осложнений различных методик уретропластики
5. Разработать алгоритм контрольного обследования в зависимости от метода уретропластики.
Оценить результаты лечения стриктур различных отделов уретры, разработать алгоритм методов их оперативного лечения в зависимости от их этиологии, локализации, протяженности, степени спонгиофиброза, оптимизировав, таким образом, результаты лечения.
Научная новизна работы:
1. Проведена комплексная оценка современных этиологических факторов возникновения стриктур уретры, согласно которым обновлены данные о патогенезе структурной болезни, выявлены прогностические факторы рецидива после операции.
2. Впервые описаны и представлены результаты новых методов уретропластики без пересечения спонгиозного тела: при коротких стриктурах – анастомотическая уретропластика без пересечения спонгиозного тела; при протяженных стриктурах - аугментационный анастомоз без пересечения спонгиозного тела.
3. Основываясь на принципах кровоснабжения уретры и его нарушении при стриктурной болезни, описана и внедрена оптимизированная операция Barbagli при пануретральных стриктурах уретры, получившая название дорзо-латеральная или односторонняя onlay методика (операция Kulkarni).
4. Обоснованы различные методы диагностики рецидивов стриктур мочеис-пускательного канала.
5. Основываясь на анализе большого клинического материала, особенностях течения стриктуры уретры в зависимости от ее причины, локализации по отделам уретры, протяженности и степени спонгиофиброза, результатах как классических вариантов операций, так и внедрения новых методов уретропластики, обновлен алгоритм выбора того или иного современного метода уретропластики с применением ...................

1. Причины возникновения стриктур мочеиспускательного канала у мужчины были: в 43.7% случаев ятрогенные, в 20.5% - поражение уретры при переломе костей таза, в 17.9% - травма передней уретры, в 8.9% - идиопатические, в 7.9% - поствоспалительные, в 1.1% - последствие BXO. Среди ятрогенных причин преобладали трансуретральные вмешательства (16.8%) и длительное дренирование мочевого пузыря уретральным катетером (11.1%).
2. Эффективность методик уретропластики составили: анастомотическая уретропластика конец в конец – 90%, dorsal onlay – 84.2%, ventral onlay – 82.4%, inlay – 80%, анастомотическая уретропластика без пресечения спонгиозного тела – 93.7%, перинеостомия – 77.8%, McАninch – 77.8%, Orandi – 83.3%, многоэтапная уретропластика – 88.2% и бульбо-простатический анастомоз – 85.7%.
3. Основополагающим в выборе метода уретропластики является этиология стриктуры. Методы уретропластики с пересечением спонгиозного тела (операция Хольцова или аугментационный анастомоз) показаны при посттравматических стриктурах уретры, когда необходимо полное иссечение зоны стриктуры и спонгиофиброза. При первичных нетравматических СУ, не связанных с BXO, показано применение методики без пересечения спонгиозного тела (анастомотическая уретропластика без пересечения спонгиозного тела или аугментационная уретропластика с использованием слизистой щеки или кожного лоскута). Операция Kulkarni - вариант выбора одноэтапной уретропластики пануретральной стриктуры, являющейся результатом BXO.
4. Осложнением анастомотической пластики уретры наиболее часто является эректильная деформация полового члена (3%). При аугментационных методиках наиболее частым необратимым осложнением является подтекание мочи после мочеиспускания: при ventral onlay – 23.5%, dorsal onlay – 21%, McAninch – 66.7%, Orandi – 16.7%. Эректильная деформация полового члена установлена нами при методике dorsal onlay в пенильном отделе в 4.5%. Возникновение de novo нарушения эрекции и недержания мочи не характерны для уретропластики передней уретры. При двухэтапной операции сморщивание графта отмечалось в 35%, что требовало выполнение дополнительной операции.
5. Разработанный алгоритм обследования и диагностики рецидива стриктуры после различных методик уретропластики кроме урофлоуметрии включает выполнение: ретроградная уретрография после анастомотической уретропластики конец в конец и бульбопростатического анастомоза; уретроскопии после onlay методик, анастомотической уретропластики без пресечения спонгиозного тела, операций McАninch/Orandi, многоэтапной уретропластики; калибровки уретры после перинеостомии.
6. Разработанный алгоритм лечения пациентов, страдающих стриктурой уретры в зависимости от этиологии локализации, протяженности и степени спонгиофиброза позволяет достичь высокой эффективности лечения стриктур. При поражении ладьевидной ямки излечение было достигнуто у 85.7% пациентов, пенильного отдела – 85.3%, бульбозного отдела – 89.3%, при пануретральных стриктурах – 78.5%.
